NOTES, Gallbladder Surgery

NOTES method is a newly developing method in the world and it is the surgical intervention of the abdominal organs by endoscopy. With this method, hernia surgery, biopsy, gallbladder removal, appendicitis surgery can be performed by entering from the stomach or intestines without any traces in the abdomen. For the first time in Turkey, in our clinic, the gallbladder of trial animals were successfully removed by entering from the stomach, it is planned to make it available for humans in near future.

Capsule Colonoscopy

PC 2 colon capsules were started to be used in the leading universities of Europe in early 2010. In Turkey, it is used in Yeditepe University Hospitals.

  • Having cameras at both ends,
  • To be able to intervene in the capsule location by observing the location of the capsules from the recording device in the waist simultaneously,
  • Being able to measure,
  • The fact that each capsule camera angle reached the maximum level (recording like a sphere) with 172 degrees is the factors that bring 90 percent success.

How Is Capsule Colonoscopy Performed?

  • The patient is given medications for bowel cleansing the night before.
  • The patient should be present at the clinic at 09:00 in the morning of the procedure.
  • After the patient definitions are made to the recording device, the recording device is attached to the patient’s belt.
  • At 11:00, the medicine deemed appropriate by the physician should be taken to regulate bowel movements.
  • At 17:00, the recording device attached to the belt is brought to the clinic.
  • After the procedure, the records are examined by the physician and a report is written.

Sedation during the procedure does not involve advancing the endoscope from the anus, and it is excreted in the stool after the capsule is swallowed. Follow up is not required. During the procedure, the patient can continue his daily activation as he does every day.

Double Balloon Endoscopy

Our clinic has pioneered the widespread and use of this method in our country and has provided training for many physicians.

Double Balloon enteroscopy has been performed in our clinic for about 4 years and more than 300 patients from our country and neighboring countries have been successfully treated, and no serious complications have occurred in any of the patients in this process. This technique, which is especially used in unexplained bleeding, should be considered in the forefront of pathological changes of the Gastrointestinal Tract (Iron Deficiency Anemia, unexplained malignity, suspected Crohn’s disease, unexplained chronic abdominal pain …)

Our hospital has the opportunity to be the first center to apply Double Balloon enteroscopy in our country.

Hemorrhoids Treatment

Findings such as rectal pain, fullness, bleeding, and itching are often signs of hemorrhoids. While the treatment of hemorrhoids is possible with various drugs at the beginning of the disease, it will be appropriate to perform surgical treatment in the most advanced period (constantly coming out of the anus). Hemorrhoids outside these two end tables can be treated with band ligation, sclerotherapy (injection with a needle) or laser. For the first time in Turkey to be used in our clinic, hemorrhoids were monitored internally during flexible colonoscopy with the infrared device, and the procedure was started while the patient was sleeping. Your hemorrhoid treatment can be done in about 20 minutes and it is possible to go home and work immediately afterwards.

Reflux Disease

Reflux disease is the leakage of stomach contents (stomach acid, food …) into the esophagus and mouth due to insufficient function of the valve between the stomach and esophagus. It can cause very different symptoms such as tooth decay, hoarseness, cough and a burning sensation from the stomach to the chest. For the diagnosis of Reflux Disease, firstly Upper Endoscopy and then 24 hour pH meter application and motility study are required if necessary. After the definitive diagnosis, medical treatment, Endoscopic Treatment (Stretta, Plicator, Gastrofix) or surgical treatment is possible. All these diagnosis and treatment methods are successfully performed in our unit thanks to our expert Gastroenterology Team.

Diagnosis and Treatment of Gastroesophageal Reflux Disease

Colon Cancer Screening

Colon cancer is the second most common tumor and prevention of this disease is possible with a simple colonoscopy. Colon cancer develops from grape-like meat pieces (polyps) hanging into the intestine, which we call polyps. The transformation of a normal polyp into bowel cancer takes 3-5 years, and it is possible to see these polyps during the colonoscopy procedure and to be removed from the intestine using special devices. Periodic colonoscopy will prevent the development of bowel cancer by cleaning the polyps. The World Health Organization recommends that all normal individuals undergo colonoscopy at the age of 50 and be followed up at regular intervals. The follow-up of risky patients (those with bowel cancer in their family, those with bowel disease …) is different and planning by consulting a physician will be appropriate.

ERCP(Endoscopic Retrograde Cholangiopancreatography)

With this method, the area where the bile duct is opened in the intestine is found and imaging is performed by entering the biliary tract and the pancreatic duct when necessary, during the procedure, if necessary, it can be performed in therapeutic procedures such as cleaning the stones in the bile ducts, placing a stent (plastic catheter) to clear the obstructions, taking biopsy. Since the day our department was established, ERCP has been carried out frequently and successfully in our hospital, and patients are sent home after an average of 2 hours of follow-up.

In our Hepatology Unit

Chronic hepatitis B and chronic hepatitis C are common diseases in our country. After the virus enters the human body, it is often passed like a mild infection and a protective substance is formed in the blood, but in some people, as a result of the body’s inability to destroy the virus sufficiently for various reasons, hepatitis B or hepatitis C virus settles in the liver and causes a disease with chronic inflammation. Although the chronic inflammation period varies from person to person, it is certain that these patients will have cirrhosis over the years if they are not treated in a timely and correct manner. Liver function tests, HBV and HCV PCR tests, upper abdominal ultrasonography and liver biopsy are required for diagnosis. After the diagnosis is finalized, Interferon and various antiviral drugs are successfully used in the treatment of chronic hepatitis B and C patients, and the full treatment rates can be up to “…..” These patients are followed up in our clinic with special forms.

Ulcerative Colitis and Crohn's Disease

These two diseases are characterized by intermittent ulcers of the intestines, abdominal pain, diarrhea, weight loss, and bloody stool, especially in Ulcerative Colitis. Colonoscopy and/or Double Balloon Enteroscopy are required for the diagnosis of the disease, and effective drugs are available for treatment.
